Click on your Office365 Hosting account in the center of the page. 
Once you click on the account, you can click the Cancel button at the top right, and fill out the cancellation form.

You can cancel your Office365 Subscription(s) from within the nvArea. Once you login, click on Hosting Services, Manage Hosting.
